IN NETWORK INSURANCE

I am an “in-network” provider for the following insurance companies:

Alliance, Dean, Quartz, and United Healthcare.

I am an “out-of-network” provider for all other insurances, so please check

with your insurance company to see if your policy includes out of network

coverage. Costs that are not covered by your insurance will be your

responsibility.

I also accept Out of Pocket contracts for those that want to use a self-pay

option. You can use your Employer Benefit plans to seek pre-tax

reimbursement for your patient care; including FSAs, HRAs, HSAs and

medical reimbursement accounts.

Fees are due in full at the time of service & each client is required to keep a

credit card on file. Sliding scale options will be made available on a case by

case basis.

GOOD FAITH ESTIMATE

The No Surprises Act is a federal law that went into effect on January 1, 2022 in order to protect you from unexpected out-of-network medical bills. Providers must give you a good faith estimate of what your care will cost. You may be able to dispute your bill if it’s at least $400 more than the estimate. You can visit www.cms.gov/nosurprises or call 1-800-MEDICARE (1-800-633-4227) for more information. 

The fees listed on this site serve as your Good Faith Estimate for your appointment. However, if you are not sure what the appointment or billing will be, please let me know prior to scheduling so that I may provide clarification or any documentation you might want.

LATE CANCELLATION/NO SHOW FEES

Your appointment time is reserved exclusively for you & I generally run a punctual practice. I understand that life happens so if you arrive within 15 minutes of session start, I’m happy to see you for the time remaining in your appointment. After this, I may choose to have you reschedule the visit due to the remaining time being inadequate for providing good care. 24 hours (1 business day’s) notice is requested for appointment cancellations or changes. If less notice (or no notice, in the case of no-shows) is given, the full appointment fee will be charged. Please keep in mind that you have the ability to manage your appointments through your patient portal. If you have any barriers that may prevent you from giving adequate notice of cancelling, please discuss this with me so we can troubleshoot and prevent recurrent charges.
